## About this clinic

Persistent pelvic pain is real, complex, and treatable.

Persistent pelvic pain (PPP) is one of the most common and most undertreated conditions in women's health. It affects 1 in 4 women, yet it is frequently dismissed, misdiagnosed, or managed in isolation. Endometriosis and adenomyosis — which affect 1 in 7 women — carry an average diagnostic delay of more than 7 years. That is 7 years of pain, uncertainty, and too often, disbelief.

Outside the Box was built in part to change this. All of our GPs have completed specialist [**Vagenius Training PPP Course** ](https://www.vageniustraining.com/course/persistent-pelvic-pain)— and Dr Emily Ware serves as Clinical Lead of this course, upskilling GPs across Australia in this area. We understand persistent pelvic pain deeply, and we treat it as the complex, whole-person condition it is.

## Our Approach

Whole-person care, evidence-informed.

Persistent pelvic pain rarely has a single cause. It is best understood and best managed through a biopsychosocial and culturally sensitive lens — one that holds space for the physical, emotional, psychological, and social dimensions of your experience. This is the framework our whole team works within.

Treatment begins with understanding the drivers of your pain — not just prescribing to the symptom. From there, we build a personalised whole-self care plan that draws on both non-pharmacological and pharmacological strategies, and coordinates a team of practitioners around you.

Biological

### Understanding the physical drivers

Hormonal, structural, inflammatory, and nervous system factors — explored thoroughly and without assumptions.

Psychological

### Holding the emotional dimension

Pain is processed through the nervous system and shaped by past experience, stress, and mental health. We address this without judgement.

Social & Cultural

### Contextualising your experience

Work, relationships, culture, and community all shape how pain is experienced and managed. Your context matters to us.

Trauma-informed

### Safety and consent at every step

You are in charge at all times. Examinations are never assumed — always discussed, consented to, and conducted with a chaperone or support person available.

## What to Expect

A 60-minute appointment that goes deep.

All pelvic pain consultations are booked for 60 minutes. This is not a rushed appointment — it is intentionally long, because your story deserves to be told in full and heard without interruption.

- **Comprehensive pain history:**
We take a thorough history of your pain — cyclical and non-cyclical, triggers, timing, character, and impact on your daily life — to begin understanding what the drivers might be.
- **Identifying your drivers:**
Using a biopsychosocial framework, we work to understand what is contributing to your pain — which may include hormonal, inflammatory, neurological, psychological, and lifestyle factors.
- **A whole-self care plan:**
We develop a personalised plan that may include non-pharmacological strategies, medications, investigations, and referrals — all coordinated across your care team.

- **Investigations if needed:**
If endometriosis is suspected, we will often organise a deep endometriosis ultrasound — a specialist scan that looks beyond standard imaging. We will explain what this involves and what to expect.
- **Pain education with our physiotherapist:**
Pain neuroscience education is a key part of managing persistent pain. We frequently organise a dedicated session with Michelle Garland, our physiotherapist, as part of your care plan.
- **Building your flare plan:**
Over time, we work with you to build a personal flare plan — a practical toolkit of strategies to help you manage pain escalations and reclaim function in your life.

#### A note on examination

A pelvic examination may or may not be recommended — this is always your choice. We practise trauma-informed care. You may have a support person or chaperone with you at all times, and you can decline any part of the examination at any point.

#### AI assisted notes

There is often a lot of information covered in this consultation. Where AI note-taking is used, we can generate a clear summary and personalised care plan that can be emailed directly to you and to your wider care team. If you prefer we don't use AI, just let us know.

## Preparing for your appointment

The more you bring, the more we can do together.

These appointments are rich with detail. A little preparation helps us use every minute well.

1. Download the [**Vagenius Period Diary**](https://outsidetheboxcare.com.au/files/images/Downloads/AAA-Vagenius-Menstrual-Diary-2025.pdf) and track your pain, symptoms, and cycle in the weeks or months before your appointment — this is genuinely one of the most useful things you can do
2. Note down your pain history — when it started, what it feels like, what makes it better or worse, and how it affects your daily life and relationships
3. Bring a list of current medications, including names and dosages
4. Bring any previous investigation results, ultrasounds, laparoscopy reports, or specialist letters
5. Consider bringing a support person — these appointments can be emotionally significant, and having someone with you can help
6. Please give us at least **24 hours' notice** if you need to change or cancel — late cancellations will incur a fee

## Team care

The evidence is clear: persistent pelvic pain and endometriosis are best managed through coordinated, multidisciplinary team care. This is not a condition for a single appointment with a single clinician. We take that seriously.

Depending on your needs, your care team may include your regular GP, our pelvic health physiotherapist, pain specialists, gynaecologists, acupuncturists, psychologists, dietitians, and other allied health practitioners — all connected and communicating around you.
