## Polyendocrine Metabolic Ovarian Syndromer (PMOS)

PMOS is a **chronic condition requiring ongoing care** — not a single diagnosis appointment. It evolves across a woman's life, presenting differently in adolescence, reproductive years, pregnancy, and perimenopause. Our approach reflects this.

We provide thorough assessment and screening, evidence-informed management, and continuity of care over time — adjusting treatment as your needs and life stage change. We also recognise the significant overlap between PMOS, neurodivergence, mental health, and insulin resistance, and address these connections rather than treating them separately.

Screening matters: 

Women with PMOS benefit from regular metabolic, cardiovascular, and mental health screening. We build this into ongoing care, not just the initial diagnosis.

 

 

 

 

 

 

 

 ###### A new name — and a more accurate one

### PMOS is a whole-body endocrine condition

The new name tells a more accurate story:

- **P** — **Polyendocrine**. Multiple hormones are involved, across multiple glands and systems.
- **M** — **Metabolic**. There is an increased long-term risk of type 2 diabetes, cardiovascular disease, and related conditions.
- **O** — **Ovarian**. Multiple immature follicles may be visible on ultrasound — but not always.
- **S** — **Syndrome**. Multiple physical and mental health symptoms are involved.

**This is why PMOS belongs in a specialist women's health setting** — one that understands the full picture and provides ongoing, evidence-informed, coordinated care rather than managing individual symptoms in isolation.

## Premenstrual dysphoric disorder (PMDD)

PMDD is not just a bad period. It is a serious, cyclical, and often debilitating condition in which hormonal fluctuations in the luteal phase of the cycle trigger profound mood, cognitive, and physical symptoms — symptoms that can disrupt relationships, work, and the ability to function in daily life.

It is distinct from PMS, frequently misdiagnosed as depression or anxiety, and too often under treated. A trained care provider who understands both the hormonal and neurological mechanisms of PMDD — and who can offer evidence-informed management — makes a significant difference to outcomes.

PMDD is a **chronic condition** — it requires ongoing management, not a single intervention. We provide continuity of care, regular review, and adjustment of treatment as your needs change over time.

Tracking is essential: 

Accurate diagnosis of PMDD requires prospective symptom tracking across at least two full cycles. ## Postnatal depression (PPND)

The postnatal period is one of the most hormonally significant transitions in a woman's life — and one of the most under-supported. Postnatal depression is common, often underdiagnosed, and can be compounded by hormonal shifts, sleep deprivation, the demands of caring for a newborn, and a loss of identity and autonomy that healthcare systems rarely address directly.

We understand that PND sits at the intersection of mental health, hormones, and the very real practical challenges of early parenthood. Effective care requires a trusted team who holds all of these dimensions — not just a checklist and a prescription.

We work closely with a trusted network of perinatal mental health practitioners, and we take the time to understand both the hormonal and the human dimensions of your experience.  ## **A note on these conditions**

Distinct from perimenopause – equally important.

A little preparation goes a long way. Here's how to get the most from your consultation:

PMOS, PMDD, and postnatal depression are often grouped with perimenopause and menopause under the broad banner of "hormonal health." They share some features — and hormones are central to all of them — but they are distinct conditions with different mechanisms, different timelines, and different treatment approaches.

What they have in common is this: they are all complex, all frequently undertreated, and all require care providers with specific training and a willingness to engage with the full picture rather than the presenting symptom alone. **Chronic conditions need ongoing care.**

PMOS and PMDD in particular are not conditions that are diagnosed and then managed with a single treatment indefinitely. They require regular review, screening, and adjustment over time. We build that continuity into the care we offer.

 

 

 

 

 

 

 

 ## Preparing for your appointment

Help us understand your experience.

New appointments are booked for 40–60 minutes, and review appointments for 30 minutes. Telehealth is available for suitable consultations.

 

 

 

1. Download the [**Vagenius Period Diary**](https://outsidetheboxcare.com.au/files/images/Downloads/AAA-Vagenius-Menstrual-Diary-2025.pdf) and track your symptoms prospectively — this is particularly important for PMDD, where accurate diagnosis requires at least two cycles of tracking
2. Note how your symptoms affect your daily life — work, relationships, sleep, mood, and function all matter
3. Bring any previous blood test results, hormone panels, or letters from other health providers
4. Bring a list of current medications and supplements, including names and dosages
5. If coming for postnatal depression support, you are welcome to bring your baby and a support person
6. Please give at least **24 hours' notice** if you need to change or cancel — late cancellations will incur a fee
