Heavy bleeding is not something to simply manage.

Heavy and abnormal uterine bleeding affects a significant number of women and can be functionally impairing — disrupting work, relationships, sleep, exercise, and wellbeing. It is also too often normalised or undertreated.

At Outside the Box, we take a different approach. Every presentation begins with thorough risk assessment — because abnormal bleeding always deserves proper investigation before treatment, and because the underlying cause shapes everything that follows. ADOLESCENCE

#### When periods first arrive

Heavy periods in adolescence are frequently dismissed as something to grow out of. They can be a sign of underlying conditions including endometriosis, adenomyosis, systemic conditions, or clotting disorders — and they are always worth investigating thoroughly. We create a comfortable, age-appropriate environment for young people to discuss what's happening.

PERIMENOPAUSE

#### When cycles become unpredictable

Changes in bleeding pattern are common during perimenopause — but not all changes are benign and none should be assumed so without assessment. Heavier, longer, or more irregular periods in the perimenopausal years always warrant a thorough review and, where indicated, investigation of the uterine lining.

We are confident combining hormonal strategies where needed to achieve better outcomes — this is not always a simple single-solution problem, and we don't treat it as one.

Including tranexamic acid, NSAIDs, and other evidence-based approaches — appropriate for women who cannot or prefer not to use hormonal options.

 ## **Endometrial biopsy**

In-clinic investigation, when it matters

For some presentations of abnormal uterine bleeding — particularly in women over 45, those with risk factors, or where the pattern raises concern — sampling of the uterine lining (endometrial biopsy) is an important part of thorough risk assessment.

 

 

 

 

 

 

 

## Procedure room

Endometrial biopsy in our clinic

Our Women's Health GPs are trained to perform endometrial biopsy in our dedicated procedure room where appropriate. This can often save women a referral to a gynaecologist and significant waiting time — without compromising on the thoroughness of investigation.

As with all procedures at Outside the Box, we practise trauma-informed care — the procedure is fully explained beforehand, your consent is always obtained, and you may have a chaperone or support person present at all times.

 

 

 

 

 

 

 

 ### **Pain management.**

We will discuss pain management options with you before the procedure. Taking paracetamol and ibuprofen approximately one hour beforehand is recommended. Penthrox (inhaled analgesia) may also be available where appropriate — please discuss this with your doctor at your appointment.

 

 

 

 

 

 

 

 ## What to expect

Assessment first, then a plan.

New appointments are booked for 40–60 minutes, and review appointments for 30 minutes. Telehealth is available for suitable consultations, though initial assessment and any procedures will require an in-person visit.

Your appointment begins with a comprehensive history and risk assessment — understanding the nature, pattern, and impact of your bleeding, your personal and family health history, and any relevant investigations you have already had. From there we develop an active management plan, not a watchful waiting approach.

**A note on AI note-taking:**

We use AI-assisted note-taking during consultations so that we can remain fully present and attentive to your health story. If you would prefer we do not use this, please let us know at any time.

Written care plans are available on request and can be emailed to you. We are happy to write to your GP on referral or on request.

 ## Preparing for your appointment

Help us understand your pattern.

 

 

 

1. Download the [**Vagenius Period Diary**](https://outsidetheboxcare.com.au/files/images/Downloads/AAA-Vagenius-Menstrual-Diary-2025.pdf) and track your bleeding pattern — cycle length, heaviness, duration, and any irregularities — in the weeks before your appointment
2. Note how your bleeding is affecting your daily life — work, exercise, sleep, social activities, and emotional wellbeing all count
3. Bring any previous blood test results, ultrasound reports, or specialist letters
4. Bring a list of current medications and supplements, including names and dosages
5. Note any treatments you have already tried and how they worked — or didn't
6. Please give at least **24 hours' notice** if you need to change or cancel — late cancellations will incur a fee
